You’ve Been Told You’re Not Anemic…So Why Are You Still Exhausted?

“I don’t understand.”

It’s something I hear from women almost every week.

“My doctor said my iron is normal.”

“But I’m exhausted.”

“My hair keeps falling out.”

“I can barely think straight.”

“I’ve started needing coffee just to make it through the morning.”

“My thyroid medication doesn’t seem to be helping like it used to.”

If you’ve ever found yourself in that situation, I want you to know something that surprises many of my patients:

You can have iron-related symptoms even if you’ve never been told you’re anemic.

One of the reasons is a laboratory marker called ferritin.

It’s one of my favorite examples of why I say we have to test—not guess.

Not because ferritin explains every symptom.

Not because every tired woman has low ferritin.

But because it’s one of the most commonly overlooked pieces of the puzzle in women’s health.


What Is Ferritin?

Think of ferritin as your body’s iron savings account.

Most people know iron helps carry oxygen through the blood.

That’s important.

But before your body can use iron, it has to have some stored away for future needs.

Those stored reserves are measured by ferritin.

Imagine your checking account and your savings account.

Your checking account pays today’s bills.

Your savings account is what you rely on when unexpected expenses come along.

Ferritin works much the same way.

It tells us how much iron your body has available in reserve.

As those reserves become depleted, your body often starts making adjustments long before traditional anemia develops.

That’s why ferritin can provide valuable information when someone is experiencing symptoms like fatigue, hair loss, or exercise intolerance—even if other laboratory values appear reassuring.


“My Iron Was Normal.”

Maybe.

But here’s where things get confusing.

Many patients use the word iron to describe several completely different laboratory tests.

They might say,

“They checked my iron.”

What they often mean is that one—or maybe two—iron-related markers were measured.

Ferritin isn’t always included.

Even when it is, patients are frequently told only one thing:

“You’re not anemic.”

Those aren’t the same conversation.

Being told you’re not anemic doesn’t automatically answer questions about your iron stores, your symptoms, or whether additional evaluation is appropriate.

That’s one reason I spend time helping patients understand what different laboratory markers actually measure instead of lumping everything together as simply “my iron.”


Why Ferritin Matters Beyond Anemia

Many people think iron only matters if someone has severe anemia.

In reality, iron plays roles throughout the body.

Your body uses iron in processes involved in:

  • Energy production
  • Oxygen transport
  • Healthy hair growth
  • Cognitive function
  • Exercise performance
  • Muscle function
  • Immune health
  • Normal thyroid physiology

That’s why symptoms related to low iron stores can show up in several different ways.

Women may tell me things like:

“I feel like I’m dragging myself through every day.”

“I used to work out all the time. Now one workout wipes me out.”

“My ponytail is half the size it used to be.”

“I can’t concentrate anymore.”

“I just don’t feel like myself.”

None of those symptoms automatically mean ferritin is the answer.

But they are certainly reasons to step back and ask whether we’ve gathered enough information before concluding that everything is “normal.”


One Number Never Tells the Whole Story

One of the biggest misconceptions in healthcare is that a single laboratory value gives us the diagnosis.

It rarely works that way.

When I review laboratory testing, I’m never looking at ferritin in isolation.

Instead, I’m asking questions like:

  • What symptoms is this woman experiencing?
  • Has she noticed increased hair shedding?
  • Is she sleeping well?
  • How are her menstrual cycles?
  • Has she recently been pregnant?
  • Has she had bariatric surgery?
  • Does she have digestive symptoms that could affect nutrient absorption?
  • Is she taking medications that may influence nutrient status?
  • What do the rest of her laboratory results show?

Healthcare works best when we connect the dots instead of chasing isolated numbers.

That’s one reason functional medicine places such a strong emphasis on understanding the whole person—not just one laboratory report.


Women Are Especially Vulnerable to Low Iron Stores

Although anyone can develop iron deficiency, women often have additional factors working against them.

Heavy menstrual bleeding.

Pregnancy.

The postpartum period.

Years of dieting.

Digestive disorders.

Certain gastrointestinal surgeries.

Reduced stomach acid.

Restricted diets.

Even endurance exercise can increase iron demands in some people.

Over time, those factors can slowly drain iron stores without someone realizing it.

Many women simply adapt.

They assume they’re getting older.

Or that being tired is part of motherhood.

Or that brain fog is just stress.

Eventually those symptoms become their new normal.

But normal isn’t always healthy.


A Patient Story I’ll Never Forget

Several years ago, I met a woman who came to me feeling completely defeated.

She had no energy.

She struggled to make it through the workday.

Her hair had become noticeably thinner.

She was frustrated because she’d been told multiple times that she wasn’t anemic.

She began wondering if everything was simply in her head.

When we reviewed her history and laboratory work together, one result immediately stood out.

Her ferritin level was 7.

That single number didn’t explain every symptom she had.

But it did identify a significant area that deserved attention and a conversation with her healthcare team about why her iron stores were so depleted.

It also reminded her of something incredibly important:

She wasn’t imagining her symptoms.

There was a reason we needed to keep asking questions instead of stopping at, “You’re not anemic.”


The Difference Between Ferritin, Iron, and Anemia

One of the biggest sources of confusion for patients is that we often use the word “iron” to describe several completely different laboratory tests.

They’re related—but they don’t all tell us the same thing.

Here’s a simple way to think about them:

  • Ferritin tells us how much iron your body has stored away for future use.
  • Serum iron measures how much iron is circulating in your bloodstream at the time your blood was drawn.
  • Transferrin is the protein that transports iron throughout your body.
  • Transferrin saturation tells us how much of that transport protein is actually carrying iron.
  • Total Iron Binding Capacity (TIBC) gives us information about your body’s ability to transport iron.
  • Hemoglobin measures the oxygen-carrying protein inside your red blood cells.

Each test answers a different question.

That’s why looking at only one marker can sometimes leave important pieces of the story unanswered.

When appropriate, I often recommend that patients discuss obtaining a complete iron panel rather than relying on a single laboratory value.


Why Ferritin Can Affect So Many Different Symptoms

Iron is involved in far more than making red blood cells.

Every cell in your body needs energy to function, and iron plays an important role in producing that energy.

When iron stores become depleted, it’s not surprising that the body may begin conserving resources.

That can show up as symptoms like:

  • Persistent fatigue
  • Exercise intolerance
  • Hair shedding
  • Difficulty concentrating
  • Feeling cold more easily
  • Shortness of breath with activity
  • Reduced stamina
  • Restless legs in some individuals

Not everyone with low ferritin experiences every symptom.

Some people primarily notice hair loss.

Others struggle with brain fog.

Others simply say,

“I just don’t have the energy I used to.”

That’s why symptoms always have to be interpreted alongside a person’s medical history, physical examination, and laboratory findings.


Ferritin and Hair Loss

Hair loss is one of the most emotional symptoms I see in practice.

For many women, it isn’t “just hair.”

It’s confidence.

It’s identity.

It’s looking in the mirror and wondering what happened.

One of the first questions I ask is:

“Has anyone checked your ferritin?”

Hair follicles are among the fastest-growing tissues in the body.

Rapidly growing tissues require adequate nutrients—including iron.

That doesn’t mean ferritin is always the cause of hair loss.

Hair shedding can also be influenced by:

  • Thyroid dysfunction
  • Hormonal changes
  • Significant illness
  • Surgery
  • Nutrient deficiencies
  • Emotional stress
  • Certain medications
  • Genetics
  • Autoimmune conditions

But ferritin is important enough that I don’t want it overlooked.

Especially when someone tells me their hair has become noticeably thinner over the past several months.


Ferritin and Thyroid Health

This is another place where patients are often surprised.

Many people think thyroid symptoms are caused only by thyroid hormone levels.

In reality, the body is much more interconnected than that.

Adequate iron is important for normal thyroid physiology, and iron deficiency may influence how efficiently the body carries out certain thyroid-related processes. Iron is a requirement for conversion of T4 to T3. T3 is the active thyroid hormone your cells use.

That doesn’t mean every thyroid patient needs iron.

It also doesn’t mean taking more thyroid medication is the answer if iron stores are low.

It simply means that when someone has persistent symptoms despite otherwise appropriate thyroid care, it’s reasonable to ask whether we’ve evaluated all of the pieces that support healthy thyroid function.

This is exactly why I encourage patients to look beyond a single lab result and consider the bigger picture.


Why Is Ferritin Low?

Low ferritin isn’t a diagnosis.

It’s a clue.

The next question should always be:

Why?

Some common possibilities include:

  • Heavy menstrual bleeding
  • Pregnancy and postpartum recovery
  • Blood donation
  • Inadequate dietary iron intake
  • Reduced absorption due to digestive conditions
  • Bariatric surgery
  • Celiac disease
  • Gut dysbiosis and infections such as H. Pylori
  • Certain medications that affect stomach acid
  • Chronic blood loss from the gastrointestinal tract
  • Endurance athletics in some individuals

Identifying the reason matters because simply replacing iron without understanding why it’s low may not solve the underlying problem.


More Iron Isn’t Always Better

Whenever patients learn about ferritin, some immediately wonder if they should start taking an iron supplement.

Not necessarily.

Iron is one of the nutrients that deserves respect.

Too little can cause problems.

Too much can also be harmful.

That’s why I don’t recommend starting iron supplementation simply because of symptoms or something you read online.

The goal is to understand whether iron deficiency is actually present, why it developed, and what approach makes sense for your individual situation.

Sometimes improving iron stores involves nutrition.

Sometimes supplements are appropriate.

Sometimes the priority is identifying ongoing blood loss or a digestive issue that’s preventing proper absorption.

The right answer depends on the individual.


Looking at the Whole Story

One of the things I enjoy most about functional medicine is putting the puzzle together.

Ferritin is rarely the only piece.

Maybe low ferritin is contributing to fatigue.

At the same time, we might also identify low vitamin B12, vitamin D insufficiency, poor sleep, chronic stress, thyroid dysfunction, digestive issues, or blood sugar imbalances.

None of those factors exist in isolation.

Your body doesn’t separate itself into neat categories.

Everything works together.

That’s why my goal is never to chase individual laboratory numbers.

It’s to understand why you’re experiencing the symptoms you’re experiencing and help identify opportunities to support better health through nutrition, targeted supplementation when appropriate, lifestyle strategies, and individualized laboratory interpretation. When prescription medications or hormone therapy may be appropriate, I encourage patients to discuss those options with their prescribing clinician.


The Bottom Line

If you’re constantly exhausted, struggling with hair loss, feeling mentally foggy, or wondering why you just don’t feel like yourself anymore, don’t assume you’re imagining it.

And don’t assume that being told you’re “not anemic” means every iron-related question has already been answered.

Ferritin is just one laboratory marker—but for the right patient, it can be an incredibly important piece of the puzzle.

Your symptoms deserve to be taken seriously.

Sometimes the next step isn’t finding a miracle supplement.

Sometimes it’s simply asking better questions.
